IV drip and rehydration in Hoi An
Two situations fill this list. The first starts with a bicycle ride to the beach and ends with somebody grey and shaking in a room near Cua Dai, having had coffee, beer and nothing resembling water since breakfast. The second is the food tour, catching up with you about two days later.
A drip is not the automatic answer to either. The doctor checks pulse, blood pressure, temperature and how much is still staying down, because for plenty of people the right treatment is oral rehydration salts, shade and a few boring hours. When fluids do go in, they go in because somebody examined you first. Blood in the vomit or stool, pain that is sharpening and settling in one spot, or confusion changes the destination: 115, or the emergency department, straight away.

An Bang and Cua Dai offer no shade worth the name between late morning and mid-afternoon. Heat exhaustion reaches us from beach clubs and from the cycle track along the river, not from the covered walking streets.
Dengue is common in this part of the country and it rewrites the plan: measured fluid balance, repeated platelet checks, no anti-inflammatory painkillers, no guessing. The doctor may run a rapid test on the spot before deciding anything.
Fluids need somewhere to hang and an hour or more of sitting still. Small homestay rooms often have neither a hook nor a chair that reclines; we bring the stand, but the room has to give up about a metre of clear floor beside the bed.
Nobody receives a vitamin drip for a hangover without being examined. If the assessment says water and salts by mouth, that is the advice you get, and we are not going to sell you the cannula instead.

Questions we are asked
Questions we are asked
Can we book a drip for tomorrow morning before our flight?
Not as a booking in that sense. A nurse can only run fluids that a doctor has ordered after examining the patient, so what you are booking is an assessment that may end in a drip. And if you are well enough to be planning a flight, oral rehydration usually does the same job and leaves your arm alone.
My partner has been vomiting since last night and cannot keep water down. Is that a home visit?
Usually yes, and it is one of the clearer reasons to call. Vomiting that blocks oral fluids is exactly what intravenous rehydration exists for. Tell us on the phone whether there is blood, severe abdominal pain, a stiff neck or a rash — any of those and we point you at a hospital instead of sending a car.
